Replacement Diplomas
Once the degree has been conferred, diplomas are mailed to students by Student Records to the address indicated in the online application (see degree conferral). Financial holds must be removed before a diploma will be produced for a graduate. A replacement diploma bears the signatures of the current University officials. Replacement diplomas will include the following notation at the bottom "Replacement Diploma"
If the diploma is damaged in the mail, a replacement will be provided free of cost if the student returns the damaged original by mail to:

Duplicate diplomas, under other circumstances, are available after a student provides sufficient proof of identity and a replacement fee of $50, payable by credit card. Duplicate diplomas will include the following notation at the bottom "Duplicate Diploma". Two valid forms of identification must be presented with a replacement diploma request:
- Driver's License or State ID
- Social Security Card with Signature
- DePaul ID
- Valid/Unexpired Passport (required for F-1 and J-1 international student requests)
Download and complete the form below for all replacement diploma requests. All faxed documents must be legible for your order to be processed. Provide a phone number where you can be reached during business hours to verify your identification. An email confirmation will be sent to you once the diploma has been mailed.
