Immunizations

DePaul University complies with Illinois Public Act 85-1315.  This law requires ALL students (both degree and non-degree-seeking) born on or after January 1, 1957, and enrolled half-time or more, to provide proof of immunization for Tetanus/Diphtheria, Measles, Mumps and Rubella.

To submit your immunization records, click on the link provided below.  This will guide you through the process of submitting your request and/or documentation.  If you are unable to submit your records at this time, you may print the form and, upon completion, deliver it in-person to either DePaul Central location.

Please note:  By law, DePaul must prevent you from registering in subsequent terms if you fail to submit documentation verifying that you meet the immunization requirements.  It is recommended that you keep a copy of the documentation submitted, as DePaul does not re-release this information.  A copy of your submission can be printed once you reach the confirmation page.
