Requirements & Form

Who Is Affected?
By Illinois law, ALL students meeting the following criteria:

  • All students born on or after January 1, 1957
  • All students enrolled half-time or more.
    • Half-time is defined as:
      • Undergraduate: 6 quarter hours
      • Graduate: 4 quarter hours
      • Professional: 6 semester hours
      • Non-Degree Seeking: 6 quarter hours

This includes all newly admitted students and all students living in residence halls as well.

What Immunizations Are Needed?
All students meeting the criteria above provide proof of immunization for the following:

  • Tetanus/Diphtheria - Within the past ten years*
    • International students are required by law to have three inoculations.
  • Measles (Rubeola) - 2 Doses
  • Mumps
  • Rubella (German Measles)

How Do You Meet The Requirements?:
Complete the IMMUNIZATION FORM and return it to DePaul Central with the proper documentation. Without a submitted form, by law DePaul must prevent you from registering for any subsequent terms. We recommend that you keep a copy of the form and records you submit, as DePaul does not re-release this information. You can submit the immunization form:

If you believe you may be exempt and have received an Immunization registration HOLD, please do one of the following:
